Contraindications:
- Do not use Spegra if:
You are allergic to dolutegravir, emtricitabine, tenofovir alafenamide, or any other ingredient of the tablet.
You are taking dofetilide (a heart rhythm medication), as co-administration is contraindicated.
Also used with caution (or avoided) in:
Patients with severe renal impairment (creatinine clearance < 30 mL/min) — limited data in this population.
Patients with active liver disease or hepatitis B/C co-infection, since liver toxicity risk may increase.
Side Effects:

Headache, nausea, diarrhea, fatigue/tiredness
Insomnia or sleep problems in some individuals
Potential Serious / Less Common Risks
Liver toxicity (especially in those with pre-existing liver disease or co-infection)
Kidney impairment in susceptible people (dose adjustment may be needed if kidney function declines)
Decrease in bone mineral density (long-term therapy)
Rare but serious — lactic acidosis, serious allergic reactions, fat redistribution/body fat changes, possible immune reconstitution syndrome when starting effective ART
Precautions:
- Must be taken exactly as prescribed — skipping doses or inconsistent use can lead to drug resistance.
Do not stop treatment abruptly; consult your doctor prior to any changes.
Need regular monitoring — kidney and liver function tests, bone health evaluation (if risk factors), overall side-effects surveillance.
Use with caution (or avoid) in pregnancy — discuss risks vs benefits thoroughly with a doctor.
During therapy: avoid other nephrotoxic drugs if possible; assess for drug–drug interactions (some medicines can reduce Spegra’s effectiveness).
Dietary Restrictions:
- You can take Spegra with or without food. Avoid excessive alcohol use, especially if you have liver disease — alcohol may worsen liver toxicity. Maintain adequate hydration, especially if you have kidney issues — this supports kidney safety.Follow safe-sex practices and avoid sharing needles/sharp instruments; Spegra reduces viral load but does not eliminate risk of HIV transmission. Regular follow-up with your healthcare provider is essential — for blood tests, adherence support, and monitoring long-term effects.
